Applications: The Department of Counseling Psychology accepts applications for Fall beginning enrollment between October 1 and December 15 for the Ph.D. program in Counseling Psychology. There is no beginning Spring enrollment for this Department. The application is responsible for collecting assembling and submitting all the pieces of the application as a complete package by the deadline.

Step 1. Apply online to graduate school
- Graduate School on-line application
- 3 Letters of Recommendation (electronically submitted)
- Application fee
- Goal Statement (Can be submitted electronically with grad school application)
Step 2. Mail supplemental materials to Counseling Psychology Department:
- Counseling Psychology Department Application Form
- 2 sets of Official Transcripts
- Goal Statement (Not necessary if electronic copy is submitted with grad school application)
Step 3. GRE and TOEFL test scores--submitted electronically directly from ETS

Criminal background checks will be conducted for all undergraduate and graduate students admitted to UW Madison School of Education programs that require field placements (e.g. practicum, student teaching, or clinical field experience). These investigations are mandated by state statute for some programs and sites and have been requested by other sites. Initial criminal history checks will be requested through the Wisconsin Department of Justice Crime Information Bureau. Costs for these checks will be borne by the School of Education.
