MMSD Photo ID Badge

Student teachers, practicum students, interns, and university supervisors who participate in and with field experience placements in the Madison Metropolitan School District must wear an MMSD Photo ID Badge daily for security purposes. The MMSD Photo ID Badges are issued yearly. If a university student or supervisor had an MMSD Photo ID last year (2007-08), the photo may still be on file. To determine whether a new photo is necessary, please contact Louise Fosdick at lfosdick@madison.k12.wi.us.

University Students

            University students placed in MMSD schools will need to contact the cooperating teacher or principal at their assigned school to obtain and sign a MMSD Student Teacher ID Badge form (available in the school’s office). Please check with the school to determine whether they process the MMSD Student Teacher ID Badge on-site or whether you will need to have it completed at the Doyle Administration Building (see below).

University Supervisors

            University supervisors with students placed in MMSD will need a Non-Employee MMSD Photo ID Badge. It is advised to have photos taken at one of your school sites and sent to the Doyle Administration Building for processing, due to the projected high number of photos needed for supervisors. University supervisors will need to present both their University of Wisconsin–Madison ID and letter of appointment from the university that indicates their supervisory responsibilities.

The Human Resources Office for the Madison Metropolitan School district is located in Room 133, at the Doyle Administration Building, 545 West Dayton Street. The office is open between 8:00 a.m. and 4:00 p.m. on school days. There may be times when the process will require the photo to be taken on one day and the ID available on a subsequent day.
