ABSENCE FROM CAMPUS
The Dean’s Office has delegated to department chairs the authority to approve absences of less than two weeks. Thus, any request to be away from campus for less than two weeks will not be sent up to the SoE Dean’s Office, but will be approved by the Chair and maintained in Diana Taylor’s office. For any absence of more than two weeks, the form must be forwarded to the Dean’s Office for approval, usually with some written justification or explanation. Approved absences also protect the individual’s claim to worker’s compensation coverage in the event of injury while away from Madison. Faculty can obtain absent forms from Diana Taylor taylor@education.wisc.edu.
AUDIOVISUAL EQUIPMENT/RESOURCES
All OT class and laboratory spaces are equipped with both voice and internet/Web access jacks. Rooms 3130, 3150 and 6225 each have integrated computer projection capabilities, VHS/DVD players, overhead projectors, built-in projection screens, and speakers for audio.. Room 6225 also has a document reader and TV and video equipment needed for distance learning.
Room 3140 houses portable equipment for use in all classrooms including: 1) four video cameras 2) seven videocassette player/recorders 3) five video monitors 4) three 35mm slide projectors and four overhead projectors; 3) one InFocus projector; 4) two video cameras; and, 5) several audio tape and replay recorders.
Several faculty/staff offices, the graduate student room and the MSOT student room all received updated computers during 2005. Internet access was acquired in early 2006 for the MS-OT student room and, just recently, Wireless Internet connection became available throughout the building. A new scanner, a fax machine for the Program office and a combination print/fax/copier for 6225 (Distance Education room) were also acquired.
There is a separate Computer/Instructional Laboratory space located in Room 3225 on the third floor of the MSC which has multiple computer work stations linked to the internet/Web, and with software to support student learning and completion of projects. The room also houses several adaptive computer input devices for teaching including Intellikeys keyboards, track balls, “Touch-free Switch”, and a graphics pad. Currently there are four Mac computers and six Dell computers and monitors in this lab. Students are also able to check out Apple and Dell laptop computers digital and video cameras from the technology library at the CIMC. This equipment is free of charge to enrolled students. We are currently broadening access to the computer lab space based on student feedback. All students have free e-mail accounts and Internet access through University of Wisconsin-Madison servers. The University provides online classroom management software that enables students to obtain course materials including the course syllabus, assignments, PowerPoint or podcast lectures, quizzes and grades from anywhere that has Internet access.
The OT Program Library, with a collection of historical and current books and journals relevant to occupational therapy practice, is located in Room 3179 and is available to students on an as needed basis. In addition, the OT program has a video/CD/DVD library in room 3140 for teaching purposes. Faculty provide students with multiple resources, including electronic readers and links to pertinent websites, through course web pages or "My UW". The UW Madison Campus is a large, well-equipped campus, able to meet the many diverse needs of its student population. Some of the resources which are particularly useful to students in our program have been the McBurney Disability Resource Center, the Multicultural Student Center, the Writing Center, International Student Services, Statistics Tutorial Service, counseling and health services, and computer support services. COMPUTER SUPPORT FOR FACULTY AND STAFF
Marc Kuklinski (262-5919) is the Information Technician for the Department of Kinesiology. He is available Monday-Friday, 10:00 a.m.-6:00 p.m. Upon request, arrangements can be made to come in earlier or stay later on a case by case basis.
Service requests should be made by e-mail with “Service Request” in the subject field. If the requested service is complex, a general explanation of the request should be transmitted via e-mail followed up by a telephone call. Service requests located at the MSC have priority on Monday and Wednesday afternoons. Service requests should be made at least 1 working day before requested service, unless it is deemed urgent. If services are needed in an urgent or emergency situation, contact Diana Taylor, and she will page Marc.
CONFERENCE CALLS
To set up your own conference call using your office phone and as many as five other dialable locations, dial *173 to connect with the conference circuit. When you hear a dial tone, dial the number of the first person. After you establish contact, depress the switchhook (or the flash button), listen for a steady dial tone, then dial the next number and keep repeating until all callers are connected. As each person joins the conference, they can talk with the group while the rest are being connected. Hang up on unanswered calls, wrong numbers, or busy signals. Your phone will ring back. Anyone can leave the conference without disrupting the connections. WisLine teleconference service is operated by the Instructional Communications systems for a fee. ELECTRONIC MAIL & NETWORK ACCESS
Activate your “My UW-MSN “account at https://www.mynetid.wisc.edu/activate. For planning purposes, please note that your UW e-mail address will be deactivated approximately two months after the end date of your campus appointment. The Division of Information Technology (DoIT) will notify you well in advance of when deactivation is going to occur. The Department of Kinesiology used e-mail as an alternative to paper memos, minutes, and other internal correspondence. Department meeting agenda and minutes will be distributed by both e-mail and hardcopy.
Wireless Internet connection is available throughout the building and in many other campus locations such as the Memorial Union Terrace.
FAX
Our program’s fax machine is located in the OT copy/mail room, 2140 MSC. The fax number is 608-262-1639. Faxes sent to the OT Program may be found on the fax machine itself, or in your faculty mailbox, if the recipient is indicated on the incoming fax. To send a fax on campus, local off campus, or long distance, please use the information in the paragraph that follows in “Telephone System”.
FEDERAL EXPRESS
If you have a package that needs to get to its destination the next day, please take your contents, along with a full mailing address, to Room 2120. The state has a contract with UPS this contract year. In order to send UPS, the program assistants in Room 2120 will need to send pertinent information (address, contents, weight) to Mary Lou Ringquist, Gym-Nat. She will prepare a UPS shipping label and will then fax (or e-mail) to us, and we will place on the package. If using Federal Express, we have funding information in our office that we can type onto a form and include with your mailing. The preferred vendor is UPS, so we will need to use this method, as long as time/staffing allows. UPS and Federal Express packages (once all labels are prepared/affixed), should be taken down to the loading dock next to the MSC mailroom. You should place your packages on a small square table, where Federal Express/UPS picks up @ 2:30 each day.
FLEET CARS
Fleet Cars. If you have occasion to use a University Fleet Car for coursework (field trip, etc.), see Mary Lou Ringquist (262-1655) several weeks before it is needed (if possible) to reserve a car. ID CARD, UW-MADISON
ID CARD, UW-MADISON
An ID card may be obtained by showing your letter of appointment and an ID photo in the Union South, Phone # 2-3258, E-mail: photoid@union.wisc.edu. Hours: Monday through Friday, 8:00 a.m.-4:15 p.m. The campus IDs are required for an e-mail address to be issued to you. Check with Steve Noftle (262-7735) to make sure you are on payroll and entered into the system before going to pick up your ID card. It generally takes 1-3 business days for this authorization to appear in the ID database. The Photo ID Office recommends a faculty/staff person call 262-3258 prior to heading to the card office to ensure their information appears in the system. The following is a list of the primary services covered by the ID Card:

Campus libraries – Your UW-Madison ID card must be presented to borrow materials from campus libraries.
Recreational facilities – The new ID cards are updated on their system nightly. SERF, Gym-Nat, The Shell, and Nielsen Tennis Stadium. There is an access fee associated with using these facilities.
Campus computer labs – These computer labs update their database with new faculty/staff info within 4 days of receipt of your UW-Madison ID. For access prior to the database update, inform the computer lab staff that you have a new card. If you have problems gaining access to labs, please contact the HELP desk at 264-4357.
NetID – To gain access to various campus services such as My UW-Madison, WiscMail, and WiscCal, the first step is to activate your NetID by going to http://www.mynetid.wisc.edu/activate/. If you have problems accessing any of these services, please contact the DoIT HELP desk at 264-4357.
Wiscard–This is the campus debit card program accepted at over 45 locations on campus. A Campus Cash account on the Wiscard is automatically set up for all faculty. To start using Wiscard, just make a deposit online; link to http://www.wiscard.wisc.edu/, by mail, in person, or over the phone.

LIBRARY RESOURCES
Students and faculty in the MS-OT Program have access to all UW Library resources.
The UW-Madison Libraries are the 11th largest research collection in North America.
The collections include more than 7.3 million printed volumes, 55,000 serial titles,
6.2 million microforms, 160 linear feet of manuscripts, and over 7 million items
in other formats, including government documents, maps, and musical scores.
Memorial Library, the main campus library is the largest library in Wisconsin. The
campus also has more than 40 subject, professional and special purpose libraries.

MAIL
Outgoing U.S. and campus mail baskets are in the OT Program Office (Room 2120 MSC). If Room 2120 is locked, you may place outgoing mail in Terri’s/Robin’s mailbox in the copy room. Mail is taken to the basement mailroom at 10:30 a.m. and 2:30 p.m. each day. U.S. mail is taken to UW Extension for postage/mailing.
OFFICE SUPPLIES
General office supplies for teaching and professional activities related to the Department of Kinesiology are supplied by the program’s S&E fund. Please contact the office supply vendor catalog (Office Depot) on the bookshelf behind the door in the OT copy room and indicate to the program office (2120 MSC) which items you’d like ordered. There is another copy of the Office Depot catalog located in 2120 MSC. For external vendors, you may submit your request via e-mail to the Program Coordinator for approval. Once you receive approval from the Program Coordinator, please forward the e-approval to the program office (2120 MSC), along with the vendor name, address, phone number, item #, cost, as well as a justification for purchase.
OVERTIME
Classified staff members may not work overtime without prior approval from Steven Noftle (MSC staff supervisor).
PARKING
Our department parking coordinator is John Hageman (262-8730) hageman@education.wisc.edu. Campus parking policies are established by the Parking and Transportation Board, composed of elected faculty members and other members appointed by the Chancellor. Since availability of parking spaces is limited, administration of these policies is difficult and complicated. Instructions are sent to employees and department sin the spring for the following year (9/1-8/31). The application process is now online. Please pay attention to details. The SoE establishes a priority list, determined largely on the basis of age, with some adjustment for rank (faculty), salary (academic staff), or pay range (classified). Special adjustments to this list may also be made occasionally based on serious and continuing physical disability or a recurrent substantial need for use of a car in connection with job responsibilities. The SoE list is merged with other campus lists, and the Office of Transportation Services determines ultimate assignment of parking spaces. Staff members who will be on a leave of absence and who wish a parking reassignment upon return from leave must notify the Office of Transportation Services. They must advise them regarding the leave period before the leave and request that their parking assignment be placed in leave status.
PHOTOCOPYING/PRINTING/SCANNING
The support staff are located in 2120 MSC. Their phone number is 262-2936. They can assist you with copying. There are two printers in 2140 MSC-one color and one black and white if you choose to do it yourself. Marc Kuklinski will set up your computer so you can choose to print from the printer in room 2140 or 3225. There is also a fax machine in that room and a scanner located upstairs in room 3225. If you need to shred documents, there is a powershred machine in room 2150.
If you need typing or copying, please submit your requests at least 2 working days in advance on the blue work order forms located on the counter in the copy room. This provides us with the necessary information to complete your job, and allows us to prioritize your job with our other work to ensure all project deadlines are met. If you have any emergency requests (work received less than 2 working days in advance), please consult with the support staff in 2120 MSC regarding the details. Larger copy jobs will be submitted to University Printing Services for copying, if time allows. University Printing Services require at least 3 working days advance notice for us to receive the flat rate, and if possible, 8 or more working days to obtain a 20% discount off the flat rate. Rush jobs under 3 days will add 20% to the flat rate of the copy job.
Due to staff request, there is an alternate work basket for support staff in the OT copy room (below faculty/staff mailboxes). When support staff offices are locked, faculty/staff have the option of placing work (with the exception of exams and any other confidential projects) in the support staff workbasket located in the OT copy room. This basket will be checked upon return to the office.
PROFESSIONAL DEVELOPMENT
Opportunities for Faculty and Academic Staff Development. The University offers many opportunities for professional development. Announcements for these frequently are posted on the website or sent to all faculty and academic staff. There is also an annual opportunity for academic staff to write a proposal for funding for professional development. This program requires departmental match for the funds requested.
PURCHASE REQUESTS
Send any 101 fund purchase requests to the program assistants in Room 2120. They will process/forward as needed. Please make sure you provide the following information: vendor name/address/phone, fax, or URL, item, quantity, cost, shipping/handling. You must also get an e-approval for all requests from the program coordinator, and submit it with your other purchase information. Items from outside vendors under $5,000 may be purchased using our department’s credit card. The program assistants in 2120 MSC will forward such requests to Mary Lou Ringquist at the Nat.
SECURITY ACCESS CARDS
If the building or area where you work or live requires an access card, you must bring a completed Access Request Form, your University ID, and one other government-issued ID such as a driver's license to the UWPD Access Card/ID Station, second floor rear (west door) of the UW Credit Union, 1433 Monroe Street, Madison. Please call 608.265.3279 for days and times for general carding. Your Department Authorizer (Room 2130) must sign your request form.
SHUTTLE SERVICE TO THE CLINICAL SCIENCE CENTER
A van shuttle service to Clinical Sciences Center generally leaves every 20 minutes from the parking area behind SMI. A schedule is posted by the SMI entrance. This service is free--you simply go out there at the appropriate time and climb in. A return shuttle leaves CSC every 20 minutes. There can be no stops on the way.

SOFTWARE ACQUISITION

A wide variety of software is available through DoIT often at extremely favorable prices; some of which may only be purchased with funds administered through the University. Effective January 2008, the School of Education is a member of the Social Science Computing Cooperative (http://www.ssc.wisc.edu).
Membership in the cooperative provides SoE faculty, staff and graduate
students with expanded access to statistical software packages and to
statistical software consulting. The membership also simplifies the
administration of statistical software licensing. For an overview of our

Login to the service is done through a Web browser using your SOE-AD

Telephone calls within the University campus require only the last 5 digits of the telephone number. (Example: Office number 262-2936 is reached by touch-toning 2-2936).
