Summer 2017 College Access Program (CAP) Application

Please fill out this application and mail in the following additional requirements: 

  • Letter of Recommendation:

    Please submit a sealed and signed letter of recommendation from a teacher, school counselor, work supervisor, or mentor (cannot be a family member or relative) that can speak to your strengths and ability to successfully complete the College Access Program. 
  • School Transcript:

    Please submit a full high school transcript. 
  • Request for Financial Aid (OPTIONAL):

    The fee to attend CAP is $500. This includes the program fee, rooming, meals, activities, etc. for the duration of the three week program. Limited funding may be available to assist families with partial scholarships. If you are applying for a CAP scholarship, please complete and submit the "Request for Financial Aid" form with your application.     **Please note that if you are accepted to the program, there is a $25 enrollment fee that must be paid. This fee cannot be waived. 

    Application materials should be postmarked and mailed by May 8th, 2017

Personal Statement:
Please write a 300-word personal statement answering two of the four following questions. Please indicate which number question you are answering in each of the statements. You may attach a word file or a pdf file for each of the questions in the section below. 

1. Please describe 3 personal and/or academic goals you hope to achieve by participating in CAP. For example, what experiences, knowledge, and skills do you hope to get out of the summer program? 

2. What special skills will you contribute as a CAP participant? (What sets you apart from other applicants? How will your attitude, personality, and experiences positively impact the summer program?)

3. Discuss your educational background, including academic strengths, weaknesses and any special needs.

4. Discuss your family background, including information like, but not limited to, the number of family members at home, their educational background, and their views about education.

    If you are unable to finance the $500 fee for the program, you may apply for a full or partial CAP Scholarship by completing an application and mailing it in to the College Access Program office. Complete a scholarship application here: Precollege Scholarship Application.
